DoDMERB (The Department of Defense Medical Examination Review Board) is the Department of Defense Agency responsible for the determination of medical qualifications of applicants for appointment to a United States Service Academy, the Uniformed Services University of the Health Sciences, the Reserve Officer Training Corps (ROTC) Programs of the United States Armed Forces, and other programs as assigned by the Assistant Secretary of Defense for Health Affairs.

DoDMERB goal is to accomplish the mission with the highest degree of accuracy, at the minimum cost for the government and the applicant, in the least amount of time.

DoDMERB staff is comprised of a cross-section of Active Duty Military and Civilian. The military is represented by members of the Army, Navy, Air Force, and Coast Guard.

The AUTHORITY: Title 10, USC 133, 3012, 5031, 8013, and Executive Order 9397.

The PRINCIPAL PURPOSE(S): To determine medical acceptability or update a medical file as part of the application process to a United States Service Academy, Reserve Officer Training Corps (ROTC) Scholarship Program, or the Uniformed Services University of the Health Sciences (USUHS).

The ROUTINE USE(S): This information may be disclosed to the Coast Guard Academy and Merchant Marine Academy for applicants to their Academies.

The DISCLOSURE: Voluntary; however, failure to furnish the requested information will impede the selection process and hamper your candidacy. Use of the Social Security Account Number (SSN) is used for positive identification of records.

NOTE #1: To allow someone access to your medical information and allow us to speak with them on your behalf, YOU must fill out this form Medical Authorization Release. Please do not return this form before you have completed your examinations.
